Laser Refractive Surgery

Laser refractive surgery is a group of advanced surgical procedures designed to correct common vision problems such as myopia (nearsightedness), hyperopia (farsightedness), and astigmatism. The goal of these surgeries is to reduce or eliminate the need for glasses or contact lenses by reshaping the cornea, the clear front part of the eye, to improve how light is focused onto the retina.

PRK (Photorefractive Keratectomy):
Procedure:
- PRK is the predecessor to LASIK and involves reshaping the cornea without creating a flap. Instead, the outer layer of the cornea (the epithelium) is removed, and an excimer laser is used to reshape the underlying corneal tissue. The epithelium naturally regenerates over a few days after the procedure.
Benefits:
- Suitable for patients with thinner corneas or those who may not be candidates for LASIK. No risk of flap complications.
Risks:
- Longer recovery time compared to LASIK, as the epithelium takes time to heal. Patients may experience more discomfort during the healing process and may take longer to achieve optimal vision.
LASEK (Laser-Assisted Subepithelial Keratectomy):
Procedure:
- LASEK is a variation of PRK, where the outer layer of the cornea (the epithelium) is loosened with an alcohol solution and then gently pushed aside rather than being removed completely. An excimer laser then reshapes the underlying corneal tissue, and the epithelium is repositioned after the procedure.
Benefits:
- Like PRK, LASEK is suitable for patients with thinner corneas and carries no risk of flap complications. It also allows for quicker recovery of the epithelial layer compared to PRK.
Risks:
- Similar to PRK, with possible discomfort during healing and a longer recovery time than LASIK.
SMILE (Small Incision Lenticule Extraction):
Procedure:
- SMILE is a newer laser refractive surgery that involves creating a small, lens-shaped piece of tissue (lenticule) within the cornea using a femtosecond laser. This lenticule is then removed through a tiny incision, changing the shape of the cornea and correcting the vision. Unlike LASIK, no flap is created in the cornea.
Benefits:
- Minimally invasive, with less disruption to the corneal nerves, which can reduce the risk of dry eyes. SMILE is particularly effective for correcting myopia and astigmatism.
Risks:
- SMILE is not suitable for all vision correction needs, such as hyperopia. As a newer procedure, long-term outcomes are still being studied.
Candidacy for Laser Refractive Surgery
Not everyone is a candidate for laser refractive surgery. An eye care professional will assess various factors to determine suitability, including:
Age:
- Candidates should be at least 18 years old with stable vision for at least a year.
Eye Health:
- The corneas must be healthy and thick enough to allow for reshaping. Conditions like keratoconus (a thinning of the cornea) may disqualify a patient.

Dry Eyes:

Glare and Halos:
- Some patients may notice increased glare, halos, or starbursts around lights, particularly at night.
Over- or Under-Correction:
- There is a possibility that the vision correction may not be perfect, requiring additional surgery or continued use of glasses or contacts.
Flap Complications (LASIK):
- In rare cases, the corneal flap created during LASIK can lead to complications such as inflammation, infection, or dislocation.
Regression:
- Over time, some patients may experience a gradual return of their refractive error, particularly if they had higher levels of myopia before surgery.
